The therapist is responsible for providingdirect clinical services to patients in the Collaborative CareModel (CoCM) in a primary care clinic, and will operate inconjunction with Optum Behavioral Care (OBC) to serve as the coremember of the hybrid in-person/virtual collaborative care team forOBC clients. In this cutting-edge team the therapist will interfacebetween patients, PCPs, and psychiatrists to support the mentalhealth and physical health care of patients on an assigned patientcaseload from OBC clients. The therapist will have the support ofOBC’s clinical and operational teams. This role will be embedded within our healthcareclient Summit Medical Group, and is a Monday-Friday role, with noevenings, weekends, holidays. PositionHighlights & Primary Responsibilities:
- Use virtual and in-personmodalities to coordinate care with the patient’s medical providerand, when appropriate, other mental health providers
- Screen andassess patients for common mental health and substance abusedisorders
- Systematically track treatment response and monitorpatients for changes in clinical symptoms and treatment sideeffects or complications
- Support psychotropic medicationmanagement as prescribed by medical providers, focusing ontreatment adherence monitoring, side effects, and effectiveness oftreatment
- Provide brief behavioral interventions usingevidence-based techniques such as behavioral activation,problem-solving treatment, motivational interviewing, or othertreatments as appropriate
- Participate in regularly scheduledcaseload consultation with the psychiatric consultant andcommunicate resulting treatment recommendations to the patient’smedical provider
- Develop and complete relapse preventionself-management plan with patients who have achieved theirtreatment goals and are soon to be discharged from the caseloadExplore opportunities at Optum Behavioral Care.

Required Qualifications:
- Meet one ofthe following license requirements for the state of
Tennessee:
- Licensed Clinical Social Worker (LCSW) , Licensed Master SocialWorker (LMSW) or Licensed Advanced Practice Social Worker (LAPSW)
- Licensed Mental Health Counselor (LMHC)
- Licensed ProfessionalCounselor
- MHSP ( LPC-MHSP) or Licensed Professional Counselor
- MHSP-Temporary (LPC-MHSP/T)
- Licensed Marriage and FamilyTherapist (LMFT) or Licensed Marital and Family Therapist
- Temporary (LMFT/T)
- 3 years of experience treating patients withbehavioral health conditions
- 1 years of experience working inprimary care or other medical behavioral integrated setting (i.e.,with other provider types)
- Experience with assessment and usingevidence-based psychosocial treatments and brief behavioralinterventions for common mental health disorders (e.g., cognitivebehavioral therapy, motivational interviewing, problem-solvingtreatment, behavioral activation
- Currently eligible to beenrolled as a provider in the Medicare program or able to terminateopt-out (if applicable) prior to starting a new position
PreferredQualifications:
- Experience in the Collaborative Care Model (CoCM)
- Demonstrated experience being detail-oriented, organized, andhave exceptional follow-up capabilities
- Proven ability tomaintain effective and professional relationships with patient andother members of the care team
- Proven ability to work withpatients in person as well as by telephone and video
- Proven solidgrasp of technology solutions and tools and ability to adapt Thesalary range for this role is $59,500 to $116,600 annually based onfull-time employment.
Pay is based on several factors including butnot limited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
