Job Summary Works in coordinationwith interdisciplinary teams to organize, prepare, and deliverevidence based clinical services across the continuum of care.
Maintains ethical principles and professional standards of practicein adherence to the discipline’s professional code of ethics. JobResponsibilities Conducts a comprehensive bio-psychosocialassessment and establishes a care plan, as appropriate.
Collaborates with the interdisciplinary Team to assess forappropriate level of care and risk factors; develops and implementscare/discharge plan and coordinates consults/referrals to removebarriers and promote optimal clinical and financial outcomes.
Utilizes specific therapeutic methods/strategies/interventions inaccordance with individualized care plan. Monitors progress ofinterventions and revises plan to meet patient/family needs.
Provides education to patient/family with respect to meetingestablished goals including but not limited to disease prevention,impact of illness and disease progression, advocacy for benefits,health maintenance, and adherence to treatment regimens. Providesdirect patient care that may include: case management, carecoordination, interdisciplinary collaboration for care transitionneeds, crisis intervention/safety planning, counseling, andemotional support to patients and families coping with illness,trauma, or end-of-life care, referral and discharge planning.
Refers patient to appropriate levels of care and availableresources that promote the individualized care plan. Maintainstimely patient-centered documentation that reflects the patient andclient systems’ pertinent information for assessment, interventionsand outcomes in accordance with regulatory licensing requirementsand third-party agencies. Performs clinical diagnosis inconjunction with conducting a comprehensive bio-psychosocialassessment and establishment of a care plan. Provides evidencebased in-home treatment to parents/infants focusing on attachmentand needs to both the caretaker and infant. Champions integratedcare model treatment guidelines in the primary care setting byproviding approximately 6-8 sessions of short-term, focusedtreatment. Independently able to provide clinical social workinterventions to all patient populations. Participates inadministrative and development activities including team meetings,clinical supervision of staff and staff education. Provide socialwork field supervision to second year social work students.
Supports the development of hospital and/or unit-based programmingto meet the clinical and strategic needs of the patients and themilieu. Works as a team member to contribute to improved patientexperience. Ensures the cost-effective utilization of resources.
Qualifications Master’s Degree in Social Work, or Counseling, orMarriage and Family Therapy or Related Human Service Field Previousexperience in a healthcare setting

Develops and maintains cultural competence with anunderstanding of the history, traditions, values, and familysystems of client groups as relates to health care anddecision-making.
