Position Status:
Exempt Responsibilities:
Manage a caseload of patients, coordinating comprehensive care from intake through discharge. Develop and oversee individualized treatment plans in collaboration with patients and the clinical team. Act as the main liaison between patients, their families, and the care team, providing education and ongoing support. Facilitate connections with external resources, including medical providers, social services, and community organizations. Conduct regular assessments, document progress, and ensure compliance with treatment plans. Participate in case reviews, team meetings, and contribute to the continuous improvement of care protocols. Address any challenges in the patient’s treatment process and advocate for their needs within the care system.
Requirements:
Education:
Bachelor’s degree in Social Work, Psychology, or a related field; a Master’s degree is preferred.
Licensure/Certifications:
Relevant licensure such as LCSW (Licensed Clinical Social Worker), LMHC (Licensed Mental Health Counselor), or LPC (Licensed Professional Counselor) is highly preferred and may be required depending on state regulations.
Experience:
Previous experience in case management within mental health or substance abuse treatment environments.
Skills:

Technical Proficiency:
