Position Administrative Requirements:
- Ensures implementation of the program as outlined by the contractual agreement with the Funder.
- Supervises Case Managers and therapists.
- Ensures fidelity to the curriculums and program requirements by conducting on going file reviews and staff supervisions.
- Ensures that Case Managers are utilizing the Strength -Based Case management tenets.
- Maintains safety standards, attends community events, reports program progress and needs to the Program Director and CEO.
- Plans and establishes realistic goals and objectives for all areas of responsibility to ensure tasks and services are effectively performed.
- Monitor program activities and program objectives to ensure efficient and effective program achievement on an ongoing basis.
- Responsible for the completion and submission of data for the monthly and quarterly reports.
- Ensure concise and timely completion of all paperwork or documentation, including but not limited to administrative requirements, clinical or case load requirements
- Assist in the implementation of clinical and administrative systems that maximize service delivery. Provide input in programmatic strategies.
- Manage relationships with Community Partners, such as schools, law enforcements, and community-based providers.
Participate in community activities, and build relationships with other community organizations, leaders and members.
Case Management Role:
- Has the ability to design and implement case management pathways for staff and protocols for treatment of selected conditions.
- Has excellent working knowledge of behavioral and evidenced based treatments for behavioral health conditions.
- Has ability to work through brief client contacts as well as to make quick and accurate assessments of behavioral and case management conditions.
- Is comfortable with pace of primary care, working with an interdisciplinary team, and has strong communication skills.

- Application of behavioral principles to address lifestyle and health risk issues
- Consultation and co-management in the treatment of psychosocial issues.
- Assist the clinical administrators in recognizing, treating and managing mental health and psychosocial issues and acts as a contributing member to the primary care team.
- Conduct client’s intakes, focusing on diagnostic and functional evaluations, then makes recommendations to the primary care team concerning the clients’ treatment goals and plan.
- Participates in trainings to enhance their skill effectiveness in treating mental health and behavioral issues.
- Provides focused intervention for clients who are in need of behavioral and case management services.
- Provides provider with timely feedback of the client needs, treatment recommendations and progress via documentation in the clients’ record and verbal feedback.
- Advises provider about which clients are better served at the agency and which should be referred to specialty mental health facilities’ or elsewhere.
- Initiates follow-up to ascertain how clients are doing and to deterring if any changes in treatment approaches are indicated.
- Develops, where indicated, relapse prevention plans and helps clients maintain stable functioning.
- Assist in the detection of “at promise” clients and in the development of plans to prevent worsening of their condition.
- Monitors and coordinates the delivery of services for client as related to behavioral care, including linking with other treatment providers.
- Assist, to the extent feasible, in the clients’ community functioning by helping with public benefits, vocational rehabilitation, social support, housing, etc.
- Documents the clients’ progress and diagnostic information in chart. Proven experience in program or project management roles with a focus on IT projects. Working conditions may include, but are not limited to:
- Office and Community based services
- Walks or stands 50 percent of the time, sits the other 50 percent;
- Works in shared groups space with moderate noise level;
- Often works independently, linked to internet resources;
- Works with frequent interruptions;
- Will be available to work a flexible schedule, evening and weekends as scheduled to meet organizational needs.
