Responsibilities Conduct patient assessments to determine needs and develop individualized care plans Collaborate with healthcare providers to ensure appropriate treatment plans are implemented Coordinate discharge planning for patients transitioning between inpatient and outpatient settings Provide counseling and support services to individuals dealing with mental health issues or addiction Utilize utilization management techniques to optimize patient care and resource allocation Work closely with clinics, hospitals, and behavioral health facilities to facilitate comprehensive care Qualifications Previous experience in case management, discharge planning, or related healthcare fields Knowledge of hospital medicine, behavioral health, addiction counseling, and mental health counseling Familiarity with utilization management practices and patient assessment techniques Strong communication skills and the ability to work effectively in a multidisciplinary team environment Certification or licensure in a relevant field is a plus
