Case Managers for Residential Treatment Facility located in Fort Myers, FL. Must be local, this is not a remote position.
Summary:
The role of a Case Manager at Celadon Recovery includes assessing individuals receiving care, establishing needs, and assisting in coordination of a treatment plan to execute services to meet identified needs. Case Managers will assist activities in planning and coordinating offsite services, plan and conduct groups, and create and execute discharge plans. Case managers oversee client’s activities within and outside of facility ensuring appointments are coordinated and met, while assisting the clinical team in preparing for discharge and arranging outside continuous care.
Responsibilities and Duties:
- Perform intake duties when needed including necessary releases of information to follow up with care, referrals, resources and communicate with parties identified for continuity of care and arrangement of proper discharge planning.
- Complete and provide staff a copy of the individual service plan and any amendments to the plan.
- Provide staff the assessment information needed to determine a resident’s eligibility and the information needed develop the individual treatment plan; meet with each resident within 48 hours of admission for orientation and review.
- Provide staff ongoing information regarding the resident’s progress and other factors which may assist in the treatment process.
- Assist the staff in relating treatment goals to the environment in which the resident will live after the completion of treatment.
- Meet with treatment team / case management conference to receive information from RTF staff regarding client’s individual treatment plan progress, and amendments to the plan, client’s progress towards treatment goals, changes in the client’s status or condition that may affect other services the client has planned for discharge.
Also, Case Manager is to receive feedback on any changes to current expectations for discharge.
- Support and closely coordinate care with client’s primary therapist and counselors for the purpose of continuity of care.
- Observe and assess clients for mental health or addiction symptoms and /or behaviors and communicate findings to staff.
- Facilitate client engagement and follow-up with specific client requests for assistance.
- Assist with treatment plan criteria related to discharge planning, appropriate referrals, family visits, recidivism studies, and all duties assigned by the clinical supervisor, including participation in group facilitation according to the outlined clinical schedule.
- Assist in facilitating outreach services for clients as needed and arrange for by clinical staff.
- Maintain accurate records and documentation in a timely fashion.
- Observe and maintain the rules and policies of Celadon recovery and conduct oneself in an ethical and professional manner.
- Meet regularly with the Clinical Supervisor for trainings and supervision as directed.
- Remain compliant with all confidentiality requirements in accordance with PHI, including but not limited to HIPA, 42CFR2 and maintain absolute confidentiality regarding residents protected health information and follow all requirements regarding completion of releases, release of specific information and rules and parameters of disclosure.
- Responsible for completing phases and passes with residents, and submit information to the team for approval, understanding the importance of reviewing the results with those residents on the case manager’s identified caseload.
Qualifications:
Obtained CPR-First Aid-AED certification Cleared Tuberculosis Testing/Questionnaire Cleared Level II Clearinghouse background check Bachelor’s Degree or Equivalent Experience (Major in Psychology or Addictions Studies Preferred) 3 years of experience with mental health and substance disorders strongly preferred; with understanding and knowledge of family systems Familiarity with the disease concept of addiction and subsequent behaviors exhibited by those afflicted
