This role is pivotal in providing high-quality behavioral health services and support to individuals and families within the community. The ideal candidate will possess a strong background in patient care, case management, and therapeutic practices, ensuring that clients receive comprehensive support tailored to their unique needs. Duties Conduct diagnostic evaluations to assess client needs and develop appropriate treatment plans. Provide home community care services, focusing on enhancing the quality of life for clients in their own environments. Deliver clinical counseling and therapy sessions, utilizing evidence-based practices to address behavioral health issues. Engage in patient assessment activities to monitor progress and adjust treatment plans as necessary. Implement applied behavior analysis techniques for clients with specific behavioral challenges. Collaborate with multidisciplinary teams to ensure holistic care and effective case management. Support geriatric clients by addressing their unique emotional and psychological needs. Maintain accurate documentation of client interactions, treatment plans, and progress reports. Requirements Master’s degree in Social Work (MSW) from an accredited institution. Valid state licensure or certification as a social worker. Experience in diagnostic evaluation, patient assessment, and case management preferred. Strong knowledge of behavioral health practices, including therapy techniques and applied behavior analysis. Excellent communication skills with the ability to build rapport with clients from diverse backgrounds. Ability to work independently as well as part of a collaborative team environment.
