Job Summary:

We seek a compassionate, organized, and dedicated Case Manager to join our team in Miami, Florida. The Case Manager will provide comprehensive support and advocacy for clients, helping them navigate and access necessary community resources and services in line with our mission to help one child, one life at a time. This role involves assessing client needs, developing, implementing, and monitoring individualized care plans, and ensuring clients achieve their goals towards self-sufficiency and well-being. The ideal candidate will have a strong understanding of the social services landscape in Miami-Dade County and a commitment to empowering individuals and families.

Key Responsibilities:

Conduct comprehensive assessments of clients’ needs, strengths, and challenges, particularly focusing on families, at-risk youth, and victims of human trafficking. Develop, implement, and regularly review individualized service plans in collaboration with clients, aligning with our person-centered care planning vision. Connect clients with appropriate community resources in Miami-Dade County, including housing, healthcare, mental health services, employment assistance, financial aid, and legal support.
