Counseling and Therapy, LLC 1001 North Miami Beach Boulevard, North Miami Beach, FL 33162 G.R.O.W.T.H Counseling and Therapy, LLC is looking for a Targeted Case Manager who is excited and passionate about helping others. The targeted case manager will be responsible for working with an assigned caseload of adults and/or children with mental illnesses or behavioral challenges by assessing their needs, planning and coordinating services within the community, linking clients to various resources, as well as advocating on their behalf and monitoring their progress and/or transition. TCM responsibilities to include, but are not limited to: Completing case management assessments, questionnaires, and developing service plans Accepting new cases in which the intake assessment has already been completed Being able to provide face-to-face and virtual interactions with clients Providing case management functions in accordance with the service plan’s problems, needs, and strategies. Being able to perform other duties assigned by supervisor that are consistent with the positive and that are incompliance with policies and procedures. Case Manager Requirements and Qualifications Possesses excellent written and spoken communication skills Excellent time management skills; organized and able to prioritize Motivated to take on additional community involvement projects and solve problems Self-directed and able to work without supervision Comfortable with building personal relationships and dedicated to helping others Empathetic and supportive with mentorship and leadership skills Bachelor’s degree in social work, psychology, sociology, healthcare or medical and related fields Certified by the Florida Certification Board or in process of becoming certified. Ability to speak Creole or Spanish is a plus, but not required.
