Older Adult Case Manager Position Available In Miami-Dade, Florida

Older Adult Case Manager Douglas Gardens Community Mental Health Center 17000 NE 21st Avenue, North Miami Beach, FL 33162 This is a targeted case manager position which focuses on providing comprehensive case management services to geriatric mentally ill and SPMI adults in a residential treatment facility. Case manager will be expected to conduct assessments, service/treatment plans, prior authorizations, and other clinical documentation. Other functions include conducting outreach, coordinating clients’ care, offering psychoeducation/supportive counseling, and providing service linkage to a maximum caseload of 36. Knowledge of Social Security/Medicare/Medicaid processes preferred. Candidate must be able to drive agency vehicle.

Position Requirements/Qualifications:

A minimum of one year experience in a case management like position, working with a consumer population appropriate to the available position is preferred. Experience can be waived for individuals with appropriate field or volunteer experience. Minimum requirement of a bachelor’s degree in social work, Psychology, or a related human services field from an accredited College or University. Bilingual English and Spanish . Successfully complete level 2 background screening and drug screening. Individual must have a valid Florida Driver’s License with clean driving history and automobile liability coverage as required by Florida law. Completion of AHCA approved mental health targeted case management training within three months of hire – Provided by the company. Attain Florida Certification Board’s Certified Behavioral Health Case Manager Certification within six months of hire -application expense reimbursed by employer.
