Yes The Case Manager Senior is responsible for: Provide linkage, monitoring, planning, and advocacy for the most appropriate mental health services for children and adults experiencing serious behavioral and emotional problems by going into homes, schools, other treatment and community based settings.
Possess a clear understanding of the service delivery system in accordance with regulations and district policy for children and adults who meet Target Case Management criteria.
Effectively maintain and manage assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served. Perform at a rate of direct service as established by division.
Assist the Coordinator with review of clinical documentation to ensure accuracy of all technical components and quality of content.
Reconcile clinical documentation submitted daily with the Service Event Records.
Assist with the referral screening process and coordination of service delivery for the specified unit
Required Education:
Bachelor’s in
Related Field, Human Services Preferred Experience:
Required•2 years•Mental Health; Or 2 years•
Related Field Required Certification/License:
BLS (Basic Life Support)
