The ideal candidate will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary resources and assistance to improve their quality of life. Responsibilities Conduct assessments to determine clients’ needs and develop individualized care plans Coordinate and monitor services to ensure clients’ needs are met effectively Provide counseling and support to clients facing challenges such as addiction, mental health issues, or homelessness Collaborate with community resources and agencies to advocate for clients and facilitate access to services Maintain accurate and up-to-date client records and documentation Requirements Experience in areas such as Medicare, motivational interviewing, mentoring, addiction counseling, child welfare, crisis intervention, behavioral health, childcare, Section 8 housing, or workforce development is preferred Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment Bachelor’s degree in social work, psychology, or a related field is required Certification in case management is a plus

Job Types:

$20.10 – $26.13 per hour Expected hours: No less than 40 per week
