Position Purpose :
To be a key team member as LVIM implements the collaborative care model of healthcare delivery, incorporating mental, behavioral, and social health into primary healthcare.
Position Summary :
The LVIM Behavioral Health Case Manager will work in conjunction with the Director of Patient Services, mental health, and primary care providers to implement a collaborative care model of healthcare delivery. Primary duties will be to assess patient needs, participate in the creation of patient treatment plans, provide patient follow-up to ensure the patient is successful, adjust the treatment plan if current methods are unsuccessful, track and document patient progress toward treatment plan goals, and be generally aware of the operations of other clinic departments. Principal Duties and Responsibilities Assess patient needs by conducting appropriate mental health assessments based on the patient’s personal health questionnaire and primary health care provider referral. Conduct, document, and track mental health assessments in EHR, including GAD-7, PHQ-9, FARS, and others. Monitor Phreesia questionnaires and follow up with patients to ensure patient needs are being met. Collect patient progress information from providers as is relevant and track patient progress toward treatment plan goals in EHR. Participate in Collaborative Care Team meetings to discuss patient progress and adjust goals as needed. Maintain the schedules of mental health providers and be willing to adjust to the needs of the patient and the provider. Communicate with the patient consistently to ensure progress is being made, to troubleshoot challenges, and to refer to additional resources as necessary. Work with employees/volunteers in other departments, including referrals, the Patient Assistance Program, the clinic, and the front desk, to ensure patient interaction and well-being. Maintain relationships and network with Tri-County, Peace River, and Empower Enter all behavioral health data into the FamCare reporting system. Compile data outcomes in regular intervals for grant reporting. Qualifications Requires a bachelor’s degree, preferably in Social Work, Psychology, Human Services, or a related field 1-2 years of experience in case management and/or a healthcare setting will be a plus Excellent listener Top candidates will be sensitive, respectful, and personable when working with others, including patients, volunteers, and medical providers Bilingual is a plus, but not required Work Requirements The position requires some lifting and filing of documents. Some local travel may occasionally be required, which will be reimbursed as applicable. Sensitive records will routinely be handled and/or observed in this position, records that fall under regulated security and confidentiality laws and regulations. Occasional after-hours events may require attendance.

Discusses volunteer HR concerns or need for a new volunteer position with the Director of Volunteer Services; Participation in LVIM’s annual fundraising and volunteer events.
