Amazing benefits, CEUs About the Organization and Position A well-known and respected non-profit in the St. Augustine area that helps children and adolescents in the foster care system is searching for an Case Manager to join their mobile team. Clients are aged 11-21 with a mental health disorder or co-occurring substance abuse issues. Therapy and wrap-around services are presented in an integrated service delivery approach that utilizes a team of individuals to comprehensively address the needs of the young person and their caregivers. Flexible schedules Clinical Supervision will be provided Responsibilities Case managers work with youth and their families to help determine what goals the family wants to achieve, and to assess their needs and goals. Then they work together with collaborative supports to achieve their goals. For performing 40 hours of work weekly, which will include flexible hours, including some evenings and weekends, according to the children’s needs. For participating in all staff meetings and treatment team meetings. For conducting the initial assessment and developing the recipient’s service plan according to Florida Medicaid guidelines. Assist with fundraising opportunities Experience To be a mental health case manager for the children, an individual must meet the following criteria: Have a bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, or a related human services field and have a minimum of one year of full time or equivalent experience working with children with serious emotional disturbances; or have a bachelor’s degree from an accredited university or college and three years full time or equivalent experience working with children with serious emotional disturbances. Has completed or agrees to complete AHCA approved mental health targeted case management training within 3 months of initially providing Medicaid services. Have knowledge of available resources in the service area for children with serious emotional disturbances. Is knowledgeable of and will comply with state and federal statutes, rules and policies that affect the target population. The ability to interface with a professional manner with the community. The ability to communicate effectively, to other members of the treatment team, difficulties that children may be experiencing.
