MSW The Roads Home Health Care Fort Pierce, FL Job Summary:
Provides psychosocial assessments, diagnosis, and treatment for patients in their homes, as prescribed by a physician, to help patients and their families develop coping skills related to the patient’s diagnosis, treatment, or life situation.
Key Responsibilities:
- Conducts comprehensive assessments of patients’ needs.
- Obtains and updates physician orders for home social work services.
- Communicates any changes in a patient’s status to the healthcare team promptly.
- Coordinates care planning and delivery with the physician, patient/family, and healthcare team to achieve optimal outcomes.
- Educates the patient/family and staff on social work interventions and resources.
- Provides appropriate social work interventions based on ongoing assessments and in line with physician orders.
- Supports a culture of safety and quality, addressing any risks to health and safety in the patient’s environment.
- Performs discharge planning and transfers in compliance with policies and applicable regulations.
- Supervises social work assistants (if applicable) and maintains up-to-date knowledge of community resources.
- Completes and submits documentation and verification of visits according to policy.
- Participates in team meetings and quality improvement activities.

Qualifications:
- Current valid state clinical social worker license.
- Master’s or doctoral degree from an accredited school of social work.
- Minimum of one year of post-master’s clinical experience.
- Preferred experience in home health.
- Valid state driver’s license, reliable vehicle, and current auto insurance.
- Current health certificate, TB test, and BCLS certification.
- Ability to communicate effectively in English; additional languages preferred.
