Job Description

MSW The Roads Home Health Care Fort Pierce, FL Job Summary:

Provides psychosocial assessments, diagnosis, and treatment for patients in their homes, as prescribed by a physician, to help patients and their families develop coping skills related to the patient’s diagnosis, treatment, or life situation.

Key Responsibilities:
  • Conducts comprehensive assessments of patients’ needs.
  • Obtains and updates physician orders for home social work services.
  • Communicates any changes in a patient’s status to the healthcare team promptly.
  • Coordinates care planning and delivery with the physician, patient/family, and healthcare team to achieve optimal outcomes.
  • Educates the patient/family and staff on social work interventions and resources.
  • Provides appropriate social work interventions based on ongoing assessments and in line with physician orders.
  • Supports a culture of safety and quality, addressing any risks to health and safety in the patient’s environment.
  • Performs discharge planning and transfers in compliance with policies and applicable regulations.
  • Supervises social work assistants (if applicable) and maintains up-to-date knowledge of community resources.
  • Completes and submits documentation and verification of visits according to policy.
  • Participates in team meetings and quality improvement activities.

  • Manages time effectively and attends scheduled patient visits.
  • Participates in continuing education and keeps skills and knowledge current.
  • Attends required meetings and training sessions and follows organization procedures for grievances, incidents, and emergencies.
  • Represents the organization positively and upholds ethical business practices.
Qualifications:
  • Current valid state clinical social worker license.
  • Master’s or doctoral degree from an accredited school of social work.
  • Minimum of one year of post-master’s clinical experience.
  • Preferred experience in home health.
  • Valid state driver’s license, reliable vehicle, and current auto insurance.
  • Current health certificate, TB test, and BCLS certification.
  • Ability to communicate effectively in English; additional languages preferred.

  • Must meet physical demands of the position, including vision, hearing, and repetitive motions.
