The Case Manager will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of social work principles and the ability to manage diverse cases effectively. The ideal candidate will possess excellent crisis management skills and a commitment to improving the lives of those they serve. Responsibilities Conduct comprehensive assessments of clients’ needs and develop individualized aftercare plans. Provide ongoing support and guidance to clients, including psychoeduccation about mental health and substance abuse when necessary. Collaborate with community resources to assist clients in accessing essential services. Monitor client progress and adjust aftercare plans as needed to ensure effective outcomes. Facilitate therapeutic and psychoeducational groups. Facilitate crisis intervention and management for clients facing immediate challenges. Maintain accurate case documentation and ensure compliance with relevant regulations and policies. Engage in motivational interviewing techniques to empower clients toward achieving their goals. Facilitate client intakes and admission processes including but not limited to bio-psychosocial assessments, risk assessments utilizing evidence-based techniques/tools, and orientation. Experience A background in substance abuse and mental health treatment, or related fields is preferred. Experience with crisis management and addiction counseling is highly desirable. Familiarity with community resource, government assistance programs, FMLA, and community housing programs is advantageous. The ability to engage and collaborate with a disciplinary team is essential. CPR certification is required. Candidates should demonstrate excellent communication skills, empathy, organization and a commitment to advocating for vulnerable populations.
