Essential Functions:

The Care Manager Associate provides coordination of health care and related services to infants, toddlers, school age children, adolescents, young adults, and their families for home & community-based services. Develops the plan of care with the client/family and interdisciplinary team and ensures client satisfaction with the services.

Job Qualifications:
EXPERIENCE QUALIFICATIONS
Required:

Case management experience in an outpatient setting.

Preferred:

1-2 years Pediatric case management experience.

EDUCATION QUALIFICATIONS
Required:

Bachelor’s or equivalent in mental or public health related field required.

Preferred:

Master’s or equivalent in public health related field, criminal justice, psychology or related field.

KNOWLEDGE, SKILLS, AND ABILITIES

Excellent people skills, using and oral/written communication. Great computer skills and the ability to learn new computer programs. Excellent documentation and time management skills, ability to prioritize important. Must be able to work as an individual contributor and also contribute to teamwork.
