Group Facilitator/Case Manager Adolescent Behavioral Health Crown Health Partners LLC 109 Oak St, Newton Upper Falls, MA 02464 At Crown Adolescent Health, we understand the unique challenges adolescents and their families face as they navigate life. Located just outside of Boston, Massachusetts, our team of dedicated professionals is here to provide compassionate care and evidence-based treatments for youths ages 13-18 who are struggling with mental health concerns. “`Overview“` We are seeking a dedicated and compassionate Group Facilitator for Adolescents to join our team of professional Mental Health providers. Hours to be determined and this is a full time position working with adolescents. As a Group Facilitator, you will play a vital role in providing support and guidance to individuals in a group setting. Your expertise in addiction counseling, mental health counseling, and case management will be essential in helping our clients achieve their goals and improve their overall well-being. “`Responsibilities“`
- Coordinate Intakes with Individuals and Families
- Facilitate group therapy sessions, ensuring a safe and supportive environment for participants
- Conduct assessments and develop treatment plans based on individual needs
- Provide individual counseling and support as needed
- Collaborate with other healthcare professionals to ensure comprehensive care for clients
- Collect and analyze data to monitor progress and evaluate the effectiveness of treatment interventions
- Assist with crisis intervention and de-escalation when necessary
- Maintain accurate and up-to-date documentation of client interactions “`Experience“` To be successful in this role, you should have the following qualifications:
- Previous experience in addiction counseling, mental health counseling, or related field
- Knowledge of evidence-based practices in group therapy
- Familiarity with data collection and coding procedures
- Strong understanding of gender issues in counseling
- Experience working in a hospital or similar healthcare setting is preferred
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
