Job Description

Qualified Professional (QP) – Mental Health, Developmental Disabilities Able To Serve Home Services, Inc 8632 Casa Court, Charlotte, NC 28215 Job Summary We are seeking a compassionate and dedicated Qualified Professional (QP) to join our team in providing community-based mental health, developmental disabilities, and substance use services. The QP will be responsible for delivering person-centered care, developing and implementing treatment plans, and coordinating services to support individuals in achieving their recovery goals.

Duties Service Delivery :

Provide direct care and support to individuals with mental health, developmental disabilities, and/or substance use challenges in community settings.

Person-Centered Planning :

Develop, implement, and monitor individualized treatment plans in collaboration with clients and their support networks.

Case Management :

Coordinate services with community resources, ensuring continuity and quality of care.

Documentation :

Maintain accurate and timely records of services provided, including progress notes and treatment plans, in compliance with agency and regulatory standards.

Crisis Intervention :

Respond to client crises, providing immediate support and intervention as needed.

Collaboration :

Work closely with interdisciplinary teams, including therapists, case managers, and other professionals, to ensure holistic care. Participate in ongoing professional development and training opportunities to stay current with best practices in the field. Educational & Experience Requirements Bachelor’s Degree in a human services field (e.g., Social Work, Psychology, Sociology) and 2 years of full-time, post-bachelor’s degree accumulated experience with the Mental Health population Bachelor’s Degree in a non-human services field and 4 years of full-time, post-bachelor’s degree accumulated experience with the Mental health population; or Master’s Degree in a human services field and 1 year of full-time, post-graduate degree accumulated experience with the Mental health population Qualifications Strong understanding of mental health, developmental disabilities, and substance use disorders. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Proficient in Microsoft Office Suite and other relevant software. Valid North Carolina Driver’s License and reliable transportation. Ability to maintain confidentiality and adhere to ethical standards.
