Foster Care Case Manager 3.6 3.6 out of 5 stars Charlotte, NC 28211 Support Incorporated is looking for a Case Manager for our Therapeutic Foster Care (TFC) and Intensive Alternative Family Treatment (IAFT) department. Minimum Education and experience:
Employee must be a Qualified Mental Health Professional; Employee must have a minimum of two years’ experience with the child/adolescent MH/SA treatment. The Agency’s preferred educational requirements for IAFT Case Manager are a Bachelor’s Degree in the Human Service field (i.e. Psychology, Social Work). There can be consideration of other degrees with special consideration given to work experience relevant to this population. The person must have a minimum of four years providing Case Management experience within the human services field. In addition, the IAFT Case Manager must be willing and able to support the fidelity of the IAFT program, work alongside NC Rapid Resource staff, Mental Health Services Standards, Exceptional Children as well as the Department of Public Instruction rules and guidelines. Job Duties for an Intensive Alternative Family Treatment (IAFT) Case Manager 1. Case Management and Coordination Develop and implement individualized treatment plans for children and families in IAFT services. Serve as the primary point of contact for families, foster caregivers, and service providers. Coordinate care between mental health professionals, schools, social services, and other stakeholders. Ensure alignment between treatment goals and available community resources. 2. Assessment and Monitoring Conduct thorough assessments of client needs, strengths, and risk factors. Regularly monitor client progress toward treatment goals through home visits, phone calls, and meetings. Document observations, updates, and changes in client behavior and needs in compliance with agency policies. 3. Support to Foster Families Provide weekly face to face supervision. Have daily contact with foster parent for check-ins, coaching, and crisis intervention. Assist foster caregivers in implementing therapeutic interventions in line with the IAFT model. Conduct training sessions and provide resources to enhance the caregivers’ skills in managing challenging behaviors. 4. Crisis Intervention Respond promptly to crises, providing stabilization and support to clients and families. Develop and implement safety plans when necessary to address behavioral or environmental concerns. Collaborate with emergency services or other agencies to ensure client and family safety. 5. Collaboration and Advocacy Act as an advocate for the child and family within schools, courts, and community settings. Collaborate with multidisciplinary teams to ensure continuity of care and support for the child. Participate in team meetings and case reviews to refine treatment plans and address barriers. 6. Compliance and Documentation Maintain accurate and timely case records, including progress notes, treatment plans, and reports. Ensure compliance with state regulations, agency policies, and IAFT program standards. Track and report outcomes to demonstrate program effectiveness and meet funding requirements. 7. Family Engagement and Education Facilitate family sessions to strengthen family dynamics and improve communication. Educate families about mental health challenges, treatment options, and available resources. Promote family involvement in the treatment process to encourage sustainable outcomes. 8. Professional Development Attend required IAFT training and maintain certifications related to the program. Stay informed about evidence-based practices, policies, and regulations impacting IAFT services. Participate in supervision, training, and professional development activities.
