Veteran Suicide Prevention Case Manager•
SSG Fox Position Summary:
A full-time Veteran position providing case management services and supports to Veterans suffering with a mental health crisis. Provides Case Management to Veterans who are in a current state of crisis and/or have been identified as at-risk based on the utilization of the Columbia Suicide Severity Risk Scale (C-SSRS). This position with coordinate with VSC coordination center staff, CIT teams, mental health providers as well as work alongside Peer Support Specialist in order to triage Veterans in crisis. Flexible hours, including evenings and weekends and an ability to travel within the service delivery areas are required. This position will be located in various regions in North Carolina across ABCCM catchment areas and will report to VSC’s Clinical Director.

- Professional Duties /
Responsibilities:
Provides Columbia Suicide Severity Risk Scale (C-SSRS) screening and other mental health screenings to Veterans. Coordinates and monitors all aspects of case management including intake, assessment, recommendations on mental health interventions, stabilization, treatment planning, referrals for adjunctive services, family meetings, group work, individual counseling, advocacy, and liaison with other community agencies. Ensures all enrolled Veterans are served as required by the grant guidelines. Works in close collaboration with Peer Support Specialists, creating a crisis response team. Documents service delivery and maintains accurate, timely documentation and is responsible for client confidentiality. Conducts extensive outreach with Peer Support Specialists and community organizations in order to identify Veterans in crisis. Partners with first responders, law enforcement, mental health providers, crisis centers and churches. Facilitates community integration and continuity of care by ensuring that the individual receiving services has enduring ties to their community and support systems particularly while in crisis or pre-crisis situations. Participate with multiple case management teams to support the member and integrated care team’s goals in a holistic approach. Transports veterans in a safe, cautious and responsible manner. Documents all activity in case file accordance with grant program guidelines and in compliance with internal policies and procedures. Participates in Outreach efforts for and represents ABCCM in a professional manner at appropriate meetings, functions and events. Coordinates across other programs and ministries within ABCCM. Participates in all appropriate staff, supervision, and training meetings. Assumes other duties as assigned by Senior Management. Identifies problem areas, and works in collaboration with Assistant Directors to address the deficiencies of staff and challenges in the community. Communicates gaps and trends to Assistant Directors and participates in VSC leadership team meetings. Assumes other duties as assigned by Senior Management. Skills /
Competencies:
Must have knowledge of clinical issues, skill in community resource development, be well organized, a self-starter, and able to work as a member of a team. Proficiency in utilizing the Columbia Suicide Severity Rating Scale (C-SSRS) with all Veterans and families encountered. Experience working with marginalized populations. Excellent communication skills, both verbal and written. Computer proficiency and ability to learn new software quickly. Must have knowledge of VA and VSC programs, understand service delivery systems, community resource development, be well organized, a self-starter, and able to work as a member of a team. Must possess a valid driver’s license, with a good driving record; ability to travel regularly within North Carolina.
Qualifications:
Prior Military experience Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus. Minimum 2 years’ experience working with persons with population served; 1-year experience if paired with Master’s degree. Relationships /

Must be ambulatory; must be able to operate a vehicle; must have visual/hearing acuity. Must be able to write legibly. Must have sufficient manual dexterity to write and use a computer and other typical office equipment on a regular basis. Must possess dexterity, stand, sit, walk, lift and carry twenty pounds for short distances.
