Scope of Essential Responsibilities:
Coordinates initial client assessment. Develops and updates Person Centered Plan. Supervises implementation of Person-Centered Plan objectives. Coordinates treatment of client and family. Schedules clients to meet required team to client ratios. Supervises team and ensures compliance with agency policy and procedures. Demonstrates a proactive approach to problem-solving. Completes required professional development. Establishes and maintains professional working relationship with parents, schools, courts, social workers, case managers and all other parties involved in the treatment process of each client. Coordinates, implements and participates all Performance and Quality Improvement activities. Communicates all maintenance, financial and staffing needs to Operations Director. Supervises, evaluates and maintains all documentation regarding client treatment and progress. Ensures team maintains 24/7 coverage of clients and families. Provides supervision of team members according to individual supervision plans. Performs other duties as requested. Demonstrates a working knowledge of COA standards. Reviews and participates in program outcome and performance improvement process.
Work and Skill Requirements:
Applicable licensure to meet state guidelines Three years’ experience working with population served Prior experience in Crisis Intervention CPR, First Aid, Seizure Management, NCI Part A Good organizational skills Prior program implementation experience Ability to be sensitive to client cultural and social economic characteristics
Other Requirements:
No criminal convictions relating to violent crimes, abuse, neglect, exploitation, drug use or drug trafficking Valid NC driver license and ability to pass all background checks Education Requirements A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, post-graduate degree accumulated mh/dd/sa experience with the population served; Holds a license or provisional license issued by the governing board regulating a human service (LCSW, LCMHC, LMFT). Must complete 20 hours of Intensive In-home training within 90 days of employment. Must complete Person Centered Planning Training. Physical Requirements Be of good physical and mental health, with no indication of active tuberculosis, communicable diseases or any other health condition that may pose a threat to clients.
