Job Description
_We are partnering with North Carolina DHHS to operationalize a statewide Medicaid Plan designed to support Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system so that they receive seamless, integrated, and coordinated health care. Within the Children and Families Specialty Plan (CFSP), and regardless of where a member lives, they will have access to the same basic benefits and services, including Physical health, Behavioral health, Pharmacy,_ _Intellectual/Developmental_ _Disabilities (I/DD) services, long term services and supports, Unmet health-related resource needs, and Integrated care management. - :•The territory for this position is Burke, Caldwell, Catawba, Cleveland, Gaston, Iredell, Rutherford, and Wilkes.
You must reside in or close to the county for which you are applying. This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

- The Manager Care Management, Foster Care (Manager I GBD Special Programs) is responsible for managing and overseeing assigned care managers and ensuring fidelity to the CFSP Care Management model which includes physical health, behavioral health, and social services.
Primary duties may include, but are not limited to: + Manages resource utilization to ensure appropriate delivery of care to members, adequate coverage for all tasks and job responsibilities. + Review all Care Plans and ISPs for quality control andprovide guidance to care managers on how to address Members’ complex health and social needs. + Ensure care managers provide Trauma-Informed Care and recognize the impact of ACEs on the CFSP population. + Coordinates service delivery to include member assessment of physical and psychological factors. + Participates in cross-functional workgroups created to maintain and develop program. + Evaluates current processes of Special Program’s support functions; recommends changes for increased efficiencies and improved outcomes. + Develops and conducts training programs for staff involved in the program. + Extracts and manipulates analytical data to present findings to relevant markets and stakeholders. + Hires, trains, coaches, counsels, and evaluates performance of direct reports.
- For the State of North Carolina, in accordance with federal/state law, scope of practice regulations or contract, the requirements are:
- + Requires an active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the state of North Carolina. + Requires a MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing. + Requires three (3) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system.
- Preferred Qualifications
- + Knowledge of resources, supports, services and opportunities required for safe community living for populations receiving in-reach and transition services, including LTSS, Behavioral Health,therapeutic, and physical health services.
+ Experience working with Children, Youth, and Families who are being served by Local Departments of Social Services through Foster Care and Adoptive Assistance programs is very strongly preferred. + At least 2 years of management/supervisor experience (with direct reports) is needed for this position. + Service delivery coordination, discharge planning or behavioral health experience in a managed care setting preferred.
