Based on experience and qualifications Key Responsibilities Conduct mental health and substance abuse triage assessments both in person and via phone Provide clinical counseling, crisis intervention, and substance abuse education Work with multidisciplinary teams to develop care plans that support patient recovery Educate clients and families on Naloxone use and Medication for Opioid Use Disorder (MOUD) Maintain timely and accurate clinical documentation in accordance with facility and compliance standards Ensure continuity of care through referrals, follow-ups, and community resource navigation
Required Qualifications Education:
Master’s Degree in Social Work

Required License:
LMSW
- Required (State-appropriate or compact license active at time of submission)
Experience:
3+ years of experience as a Master’s-level Mental Health Social Worker
- Required Triage experience
- Required Working knowledge of Naloxone and MOUD

Required Other Requirements:
Must have a reliable personal vehicle
- Required All medical/occupational health compliance documentation must be completed and uploaded within 5 days of offer acceptance
