Full Job Description Position Summary:
Behavioral Health Case Manager (BHCM) is responsible for coordinating and supporting mental health care within Elliot Behavioral Health Services (EBHS). The BHCM provides clinical case management support with high acuity patients across the lifespan (including children, adolescents, adults, and geriatric) and their families. The BHCM will be assigned to a high acuity treatment team within EBHS to support an average of 40 patients. Case Management will include coordination of treatment needs, developing, monitoring, and evaluating treatment plans and progress. Also provides services that empower patients to take an active role in their treatment to meet their goals as independently as possible. Behavioral Health Case Manager works with patients to maximize emotional, social, and physical wellbeing and the appropriate and efficient use of health care services across the continuum. Promotes efficient coordination of high acuity patient care through collaborative practice with the Behavioral Health high acuity treatment team. Frequent interactions and team-based care will include psychiatrist, psychiatric nurse practitioner, therapists, primary care providers, triage nurses and other specialists. The BHCM will enhance patient, family, staff and physician satisfaction through coordination and continuity of care. Coordinates patient care through a continuum of care model thereby facilitating the achievement of optimal quality outcomes in relation to clinical care and cost effectiveness.

COMMUNICATION
Educates and supports transition of patients to community-based services. Acts as a resource to Elliot Behavioral Health providers to coordinate and respond to patient referrals on a timely basis. Communicate effectively with a variety of individuals representing diverse cultures and backgrounds and functions professionally in situations which require a high degree of sensitivity, tact, and diplomacy. BHCM role is primarily remote and employee must utilize all designated communication tools to collaborate with EBHS team. Home office supplies will not be provided by Elliot Health System.
INFORMATION
Utilizes proper systems for appointment scheduling and updates. Maintain knowledge and stay abreast/current with mental health needs and social services within the New Hampshire community.
OPERATIONS
Monitors assigned high acuity patients mental status, SDOH needs, and case management status for transition to community provider or other resource. Prepare related reports including but not limited to outcomes, successes, areas of growth, etc. Develop effective working relations and cooperate with patients’ medical team. Participate in weekly team meetings on-site at EBHS. Duties may be performed off site as determined and approved by supervisor

ESSENTIAL JOB FUNCTIONS
Ability to assess crisis situations and respond/refer appropriately in a timely basis. Assists high acuity patients with transition to a community behavioral health provider which may require scheduling appointment with patient and when necessary, support the patient’s completion of intake paperwork. Works with providers to accommodate patient referrals and provide information necessary to clients to schedule appointments with community providers. Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient centric. Assess, plan, implement, monitor, and evaluate actions required to develop and meet the patients’ health care goals and needs. Administer screenings for patients for mental health and substance abuse disorders. Provide patient education and facilitate referrals regarding common mental health/substance abuse disorders and the available treatment options. Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options. Conduct crisis intervention as needed. Develop effective working relations and cooperate with patients’ medical team. Assess and address motivational, psychosocial issues, and social determinants of health. Coordinate services within SolutionHealth care team and other health care providers Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, Solution Health guidelines. Attend patient-related meetings, program-related community meetings, and committee meetings as assigned.
