The Position:

Under the supervision of the Director ofBehavioral Health, the Licensed Behavioral Health Clinician is alicensed mental health professional providing mental healthdiagnostic assessments, treatment, consultation, and crisisinterventions for participants with a broad range of behavioralhealth needs. The Licensed Behavioral Health Clinician collaborateswith the primary care provider, psychiatrist and the IDT indeveloping and implementing plans of care. The salary range for the Licensed BehavioralHealth Clinician position at Center For Elders Independence is$75,241

  • $ 125,403 per year. Salary based on the market forLicensed Behavioral Health Clinician as well as experience, skills,abilities and work history.
  • Please note: This position willinitially support only our Concord location.
  • Once our new site inEl Sobrante opens (in/around summer 2025), the person holding thisrole will also work in that Facility as well
  • General Duties andResponsibilities:
  • In collaboration with the psychiatrist and otherlicensed members of the IDT, the
Licensed Behavioral HealthClinician:
  • Identifies participants in need of clinical servicesbased on acuity/need across all centers.
  • Develops and acceptsinternal referrals from CEI providers and/or IDT for clinical andconsultation services based on assessed need across allcenters.

Conducts diagnostic assessments to make recommendations toIDT, and develops a treatment plan of care for behavioral healthservices.

  • Provides crisis intervention services and monitorsparticipants during psychiatric hospitalizations, consulting withhospital staff during discharge planning.
  • Conducts individual andgroup psychotherapy for those with behavioral health treatmentneeds. Determines level and frequency of services and the dischargeplan to center social work team for continued support as needed
  • Makes appropriate referrals to outside behavioral health vendors(e.g. Intensive Outpatient Program, Substance Use Treatment, etc.)and monitors use of services, consulting with agency on participantprogress and discharge planning.
  • Manages assigned panel ofparticipants with behavioral health needs. Maintains documentationof participant behavior, interventions, conditions and outcomesthat affect behavioral health and prepares reports in theelectronic health record to care/treatment plans and relateddocuments.
  • Facilitates and conducts meetings with participants,family members and caregivers to provide advice on how to supportbehavioral health interventions and care plan objectives.
  • Mayinterview participant’s family members and caregivers to assesshome environment, individual functioning, family dynamics andcontributors to behavioral health concerns.
  • Collaborates with theprimary care team and CEI Social Workers to provide ongoingbehavioral health support and interventions to participants
  • Participates on interdisciplinary team (IDT) meetings once a monthto assist with the development and implementation of acomprehensive, integrated plan of care, and monitors progresstoward objectives by evaluating behavior and communicatingparticipant changes to team members.
  • Leads and facilitatesbehavioral health psychosocial rounds at centers for case review,identify interventions and or staff development for similar cases
  • Develops and maintains working relationships with other communityresources such as Department of Social Services, psychiatricfacilities, hospitals, social agencies, and other behavioral healthresources and may participate in hospital and/or SNF dischargeplanning as it relates to behavioral health needs.
  • Trains andsupports staff to increase competencies in serving participantswith behavioral health needs.
  • Trains and supports caregivers toincrease competencies in serving their loved one with behavioralhealth needs.
  • Occasionally assess potential participantsinterested in CEI with significant behavioral or mental healthbackgrounds for appropriateness to PACE based on standardizedhealth and safety criteria. Make recommendations for approval ordenial of enrollment accordingly.
  • Makes visits to variouslocations to meet with participants and/or service providers,including but not limited to, participant’s homes, hospitals andskilled nursing facilities.
  • Maintains the confidentiality of allcompany procedures, results and information about participants,clients or families in conformance with HIPAA principles.
  • -Performs related duties as assigned.
QUALIFICATIONS:
  • Master’sdegree in Social Work, Marriage and Family Therapy, or ProfessionalClinical Counselor; or Doctorate in Clinical Psychologist; (LCSW,LMFT, LPCC, or PhD license required);
  • 2 years’ experienceproviding diagnostic assessment, treatment planning, crisisintervention and psychotherapy required.
  • One year experienceworking with a frail and/or elderly population with HealthPsychology model preferred.
  • Demonstrated ability to work in amulticultural and multidisciplinary team environment.
  • Ability towork independently.
  • Excellent oral and verbal communication andmeeting facilitation skills.
  • Experience working with severe mentalillness and substance abuse is desirable.
  • Bilingual ability highlydesirable (Spanish, Korean, Cantonese, and/or Mandarin).
  • ValidCalifornia driver’s license, acceptable driving record andinsurance with access to reliable transportation.
