Company Overview Interborough Developmental and Consultation Center (IDCC) is a non-profit outpatient behavioral health clinic established in 1974. With multiple locations across New York, we are dedicated to providing culturally competent, evidence-based mental health services to individuals and families in our community. Summary We are seeking a dedicated Case Manager to join our team Home-Based Visiting Program at Interborough Developmental & Consultation. In this role, you will be instrumental in coordinating care for clients, ensuring they receive the highest quality of mental health services tailored to their individual needs. Your contributions will directly support our mission of enhancing community well-being. Responsibilities Conduct comprehensive assessments to determine client needs and develop individualized care plans. Coordinate services across various providers and community resources to ensure continuity of care. Monitor client progress and adjust treatment plans as necessary based on ongoing evaluations. Provide support and guidance to clients and their families throughout the treatment process. Facilitate communication between clients, healthcare professionals, and family members. Document all interactions and maintain accurate records in compliance with regulatory standards. Participate in regular team meetings and training sessions to enhance service delivery. Requirements Bachelor’s degree in social work, psychology, or a related field; Master’s degree preferred. Experience in case management or behavioral health settings is highly desirable. Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds. Excellent organizational skills and attention to detail in documentation and reporting. Knowledge of community resources and mental health services available for clients. Ability to work collaboratively within a multidisciplinary team.

SALARY RANGE

$60,000 -$65,000 (Based on experience and licensure/certification.)
