Monday – Friday, 8:30a-4p.

Job Summary:

The Targeted Case Manager for the Certified Community BehavioralHealth Center (CCBHC) plays a key role in improving client healthoutcomes by addressing behavioral health, mental health, and socialdeterminant needs. This position supports clients by coordinatingclinical and non-clinical services and helping them navigate thesesystems effectively. The CCBHC employs a high-quality, consistentset of protocols for screening, intake, outreach, navigation, andcare coordination, ensuring clients receive the necessary resourcesto meet their recovery goals.

Required Qualifications:

High SchoolDiploma or GED required. Minimum 1 year of related experience,preferably in Behavioral Health. Proven ability to establishsupportive client relationships. Ability to perform duties in astable, consistent, and predictable manner. Valid NYS Driver’sLicense in good standing.

Preferred Qualifications:

Associatedegree in a Health Care-Related Field.

Required Skills andCompetencies:

Communication:

Excellent written and verbalcommunication skills. Ability to effectively communicate withculturally diverse individuals, staff, and service provideragencies.

Knowledge & Experience:

Familiarity with local behavioralhealth systems and resources addressing social determinants ofhealth. Experience with MS Excel and strong data collection andentry skills.

Problem-Solving:

Strong organizational, timemanagement, and problem-solving skills. Ability to remain calm andeffective in crisis situations.

Interpersonal:

Skilled inestablishing supportive, collaborative client relationships.
