- Remote Job Overview SSGi is seeking a dedicated and compassionate RN Certified Case Manager to join our healthcare team.
The ideal candidate will provide high-quality care to behavioral health active duty personnel, veterans and their family members. These patients suffering from mental health disorders in a hospital setting. This role requires a strong understanding of case management nursing principles, excellent patient service skills, and the ability to work effectively in a fast-paced environment. The RN Certified Case Manager will collaborate with a multidisciplinary team to develop and implement individualized treatment plans, ensuring the best possible outcomes for our patients. Coordination of lots of discharges, with a plan once they are discharged

Bachelors Degree in Nursing from ACEN or CCNE accredited school
Experience:
1 year as a certified case management experience within last 2 years

- Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.
- Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.
- Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM
- Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
- Integrate CM and utilization management (UM) and integrating nursing case management with social work case management.
- Maintain liaison with appropriate community agencies and organizations.
- Accurately collect and document patient care data.
- Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
- Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.
- Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
- Assist with coordination and scheduling of Command Directed Behavioral Health evaluations
- Assist with overall utilization assessments for the Department of Behavioral Health, to include pulling data to provide for Command briefs Skills Strong knowledge of hospital medicine and medical terminology relevant to psychiatric care.
Excellent patient service skills with the ability to communicate effectively with diverse populations. Experience coordinating with Emergency Room or Critical Care settings as a Case manager for patients Experience coordinating with Pediatric Intensive Care Unit (PICU) as a Case Manager for patients Ability to remain calm under pressure and respond effectively in emergency situations. A compassionate approach towards patient care with an emphasis on empathy and understanding. Strong organizational skills with attention to detail in documentation and reporting.
