Behavioral Health Case Manager Community Health Clinic, Inc. 943 4th Avenue, New Kensington, PA 15068 The Care Manager provides professional coordination of medical and mental health services according to the individual’s level of need. The incumbent works as a member of the behavioral health and medical treatment team, often taking the lead role, and using considerable discretion and independent judgment in order to promote individuals’ recovery. The Case Manager serves as a key member of the treatment/service team, assuring often complex services produce positive outcomes. 1. Develops relationships with the individual, his/her family and other important people in his/her life as identified and with consent of the individual served. 2. Assesses individual and family strengths and needs in a collaborative method through individual and/or collateral interviews and reviews of social and clinical information provided by other entities. 3. In close collaboration with the consumer, family members, medical and behavioral health staff, promotes service planning efforts which result indeveloping, documenting and implementing a comprehensive service plan driven by the individual utilizing all the agreed upon strengths and needs. 4. Works with the Primary care team in ensuring patients Behavioral Health needs are met. 5. Works with the clinical team in managing patients receiving Medication Assisted Treatment. 6. Refer patient to therapists in the area as needed and follow up with patients and providers on progress. 7. Advocates for and with the individual being served to ensure responsiveness from natural, community generic and specialized services/supports. Advocacy includes providing information, removing barriers, creating options and resolving problems. 8. Attends training programs as provided through the State, County and Agency to assure that the incumbent is up to date on new approaches, best practices and recovery oriented services. 9. Maintains an accurate record of care management activity in the Electronic Health Record.
