Job Summary and Qualifications Provides clinical services in accordance with scope and license, to patients and their families including but not limited to assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family and group therapy.

Major Responsibilities:

Knowledge, Skills, Abilities, Behaviors

  • Demonstrates advanced clinical skills and ability to plan and facilitate individual, group and family psychoeducational and process groups
  • Identifies cultural considerations, gender identity and preferred language for patients and family
  • Demonstrates advance practice in assessments, group notes and all other documentation timely and in compliance with requirements (as appropriate to scope)
  • Continuously assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patients condition
  • Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)
  • Recognizes and acts upon unexpected assessment findings
  • Develops treatment plans and attends patient treatment planning meetings and reviews on an ongoing basis.
  • Leads in ensuring a safe structured milieu with evidence based treatment modalities/resources
  • Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers
  • Provides education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes. Assists with documentation as required.
  • Assesses, collaborates, coordinates and provides guidance on complex discharge planning needs and readiness for discharge with the multidisciplinary team.
  • Evaluates re-admissions for gaps in discharge planning processes, formulates a plan and implements new strategies
  • Develops and maintains a community resource guide including access to public assistance/alternatives.
  • Facilitates and participates in Community Education, Activities and Support Groups (as appropriate to scope)
  • Conducts post-discharge follow-up if indicated
  • Assists with Process and Quality Improvement initiatives
  • Delivers patient care based on accepted standards of care
  • Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.
  • Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts.
  • Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families.
  • Serves as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services
  • Performs utilization review as assigned
  • Participates in developing department goals/objectives and clinical programming
  • Reviews, coaches and assists with training of new staff/interns as assigned within approved practice and program guidelines.
  • Demonstrates knowledge and role in hospital codes

Education & Experience:
  • Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) Required
