•With our Agency’s mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina. This position is located at the Department of Mental Health, Central Office, 2414 Bull Street, Columbia, SC 29201. As the Mental Health Professional II working under routine supervision of the Transition Specialist Program Coordinator for the Office of Transitions Programs, identify and execute activities that result in patient discharge from inpatient settings to integrated community residences and community-based behavioral health services, to include discharge plan development, accessing residential setting, patient assessment and linking to community-based providers. Will maintain a reduced caseload and coordinate the activities of other mental health professional (MHP I’s) clinicians. Responsibilities include but not limited to:
Quality Management:
Responsible for conducting CMHC Initial Clinical Assessments and DLA20s with patients choosing CMHC for outpatient provider to support continuity of services from inpatient to outpatient settings; coordinates and locates community-based residential settings options, based on patients’ discharge plan; collects and reports data for performance improvement; Utilize the Avatar and EMR to document services provided; complete accurate and timely reports to management as required.
Leadership:
coordinates staffing with CMHC regarding program placement for patient when discharged; provides face-to-face and/or video introduction of patient to community providers to include care coordinator and peer support specialist; attends treatment team meetings, discharge committee meetings, patient staffing’s and discharge planning staffing’s to provide community-based resources and/or options for discharge planning. Serves as team lead and the first point of contact for managing workload, addressing, and responding to the needs of peers; oversees daily operations. Provide leadership, support, and guidance to other transition specialists daily. Performs assignment of duties at the delegation of management.
Initiative:
identifies, collects and routinely updates an inventory of integrated community-based residences for Transition Team distribution/resource directory; brokers needed community resources/services and develops resources where gaps exist;
Skill Enhancement:
completes core competency trainings (such as SCEIS, MAPS, etc) and other job-related trainings as required or assigned; will maintain professional licensure (as applicable) and maintain knowledge of and enforce policies and procedures.
Flexibility:
performs other job duties as required or assigned. Minimum and Additional Requirements A master’s degree in guidance, counseling, the social sciences or a related field and professional experience in a mental health, human service or social service program. Preferred Qualifications Six (6) months to one (I) year experience in a clinical program. Additional Comments Must possess ability to access and coordinate interaction between multi-disciplinary staff when needed. Must have thorough knowledge of methods, principles and practices of clinical counseling and education services. Extensive knowledge of psychopathology and patient management within a psychiatric setting. Ability to problem-solve and utilize a Cognitive Behavioral Therapy approach to patient transition. Must possess laptop computer skills, able to utilize a web-based database system of reporting, be highly organized and able to communicate effectively, both orally and in writing. This position is located in the Office of Transition Programs but will work in assigned mental health location and requires the ability to bend, stoop, climb, drive from location to location, lift and file; must perform duties in multiple settings.
