Chaplain | Hospice Healthcare Community Hospice and Palliative Care | an Alivia Care Company Jacksonville, FL The Chaplain provides spiritual/pastoral services and counseling to patients, familly members, and caregivers, in accordance with the patient’s plan of care. S/he acts as a trusted liaison for patients and family members provided their identified faith community. In addition, the Chaplain provides spiritual/pastoral counseling to various Community Hospice staff members and hospice volunteers.
Position Details Compensation:
$58,000 – $62,000 base salary plus paid driving mileage; full benefits package including 23 days PTO annually

Required:
Must have completed a minimum of four (4) units of ACPE Clinical Pastoral Education Credits (see www.acpe.edu) Responsibilities Provides spiritual/pastoral assessment within organization established timeframes and as indicated and appropriate. Provides spiritual/pastoral support in accordance with on-going wishes and needs of the patient/family including religious services, prayers, worship and/or rituals for patients/caregiver as indicated in the plan of care and in keeping with their beliefs. Develops individualized spiritual/pastoral plans of care which demonstrates an effort to work in close collaboration with local clergy whenever desired by the patient; and provides spiritual support as needed and defined by the patient/family/caregiver. Conducts religious services of prayer, worship and rituals for patient/families, as appropriate. Offers patient/families/caregivers of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs and values. Documents counseling and all patient/caregiver interactions provided in the patient’s electronic medical record (EMR) according to Community Hospice policy. Works with other professionals in resolving spiritual/pastoral/ethical issues.
Education & Required Licenses/Certifications:
Board Certified Chaplain (or eligible) with the Association for Professional Chaplains or the National Association of Catholic Chaplains, or the National Association of Jewish Chaplains, or the Canadian Association for Spiritual Care. Master of Divinity with at least 72 credit hours from a seminary accredited by the Council for Higher Education Accreditation (www.chea.org). Completion of a minimum of four (4) units of ACPE Clinical Pastoral Education (www.acpe.edu), preferably including one to two CPE units completed as a fulltime chaplain resident. Ecclesiastical Ordination or (Commissioning with Endorsement for Chaplaincy)
Desired Experience & Knowledge:
Five years (minimum) experience in pastoral care, with at least one year in a health care setting; experience in hospice or palliative care preference given. Sound hospice care and end-of-life philosophy. Standards of practice and ethical standards for practice of the pastoral care professional. Various religious/spiritual beliefs, traditions and practices, particularly as they pertain to the delivery of end -of-life care.
