$50.8K – $62.2K a year 3 hours ago Qualifications Hospice Hospice care 5 years Master’s degree Theology Senior level Care plans Full Job Description Jacksonville, FL 32257 The Chaplain (PRN) provides spiritual care services for patients/families during business or non-business hours on weekdays or weeknights and weekends; provides spiritual support for patients and/or families when requested; contributes to the smooth function of services for a 24 hour/7day a week hospice care program.
Primary Responsibilities:
1. During non-business hours, on weeknights and weekends, responds to requests from Triage team or Spiritual Care Manager to provide spiritual support for patients/families in unpredictable and/or unknown environments. 2. During business hours, on work days, respond to requests by Spiritual Care Manager to provide spiritual support for specific clients who have requested chaplain services, in unpredictable and/or unknown environments. 3. Provide chaplain coverage for Interdisciplinary Care Teams, including reporting on spiritual care provided, as reported by staff chaplains, and contributing to the update of Care Plans, as pertaining to spiritual care needs of clients. 4. Provide direct services to patients and families in unpredictable or unknown environments, including: a) Provides spiritual care in keeping with patient/families belief system. b) Makes direct contact with the clergy and/or faith community of the patient with patient’s permission, in order to facilitate requested rituals or support for specific religious/spiritual traditions and practices. c) Conducts religious services of prayer, worship and rituals for patients/families as appropriate d) Assists families at the time of a patient’s death with contacting of the funeral home, disposal of narcotic medications, and with closure rituals, if requested. e) Makes referrals to Triage team and caseload team members for appropriate follow up. f) Serves as an advocate for patient/families. 5. Document all patient/family interventions according to Hospice policy. 6. When working during non-business hours, communicate with Triage team and Spiritual Care Manager about requests received and interventions provided. 7. Comply with professional, ethical standards in delivering services to patients and families. 8. Other duties as assigned.
Education/Experience:
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to: Master’s Degree from an accredited seminary or school of theology. Minimum five (5) years experience in pastoral care, at lease one (1) year of which was in health care setting.
Knowledge of:
The hospice philosophy and end-of-life care. Standards of practice and ethical standards for practice as a pastoral care professional.
