Full Job Description Job Summary:
The Hospice Chaplain / Bereavement Counselor provides spiritual and emotional care to terminally ill patients and their families as well as grief support to the family after the death of the patient. Ability to work with people of various religious faiths and cultures. Responsible for developing bereavement programs and overseeing current programs, including social activities and workshops. Key Responsibilities Conduct a spiritual assessment within 5 days of hospice admission as part of the comprehensive assessment. Help develop and implement the patient care plan, specifically addressing spiritual care needs. Provide spiritual counseling and support according to the patient’s and family’s ongoing wishes, religious beliefs, or personal definition of spirituality. Collaborate with local clergy or spiritual leaders when requested by the patient or family. Maintain accurate written records, including initial evaluations, care plans, and visit notes. Evaluate the effectiveness of spiritual counseling and adjust care as needed. Coordinate weekly visit schedules with Case Managers to align with other care staff. Participate in Interdisciplinary Group (IDG) and Quality Assessment and Performance Improvement (QAPI) meetings and activities. Conduct religious services, prayers, worship, or rituals for patients and families as appropriate. Collaborate with other professionals to address spiritual and ethical issues. Provide spiritual and bereavement support to staff when needed. Offer consultation and education to the care team on spiritual care topics. Conduct or assist with funeral or memorial services when requested. Engage in continuous professional development through education, workshops, conferences, and independent study.
QUALIFICATIONS
Ordained, commissioned or credentialed according to the practices of an organized religious group, and has completed one (1) unit of CPE or has a minimum of a bachelor’s degree with emphasis in counseling or related subjects and has within 90 days of hire, completed specific training to include: common spiritual issues in death and dying, belief systems of comparative religions related to death and dying, spiritual assessment skills, individualizing care to patient beliefs, and varied spiritual practices/rituals. OR Has a degree in theology or from a seminary along with emphasis in counseling or related subjects and has within 90 days of hire, completed specific training to include: common spiritual issues in death and dying, belief systems of comparative religions related to death and dying, spiritual assessment skills, individualizing care to patient beliefs, and varied spiritual practices/rituals. Comfortable in an interfaith setting. Hospice experience preferred. Able to accept different lifestyles, cultures, beliefs, and values. Must be a licensed driver with an insured automobile in good working order. The ability to communicate well, both verbally and in writing.
