Job Summary:
The Spiritual Care Coordinator / Chaplain is responsible for the provision of spiritual care services to patients /families/ caregivers of Cardinal Hospice. The Chaplain coordinates spiritual care with the IDG, the patient/family and other care team members.

Job Duties Include:
- Performs spiritual assessment on all Hospice patients.
- Acts as liaison between Hospice and the spiritual community.
- Responds to the mental, spiritual, social and emotional needs of terminally ill patients and their families.
- Provides nondenominational spiritual counseling per patient and/or family request.
- Functions as a member of the interdisciplinary group.
- Provides emotional support through discussion of spiritual and/or existential issues.
- Assists family in understanding the grief process.
- Gives referrals to family for community grief support groups and maintains communication with community chaplains and other spiritual counselors to aid patients and families.
- Allows patient and family to voice their concerns, fears and feelings.
- Serves as liaison and support to community chaplains and spiritual counselors.
- Addresses and supports cultural practices, as long as such practices do not harm others.
- Supports a culture of safety and quality.
- May perform specific religious duties related to the faith in which he/she is ordained, i.e., administration of last rites and provides support for funeral or memorial services for patients as requested.
- Understands Hospice philosophy and issues of death and dying.
- Accepts and performs other related duties and responsibilities as required.
- The ability to work effectively in stressful situations while maintaining a calm demeanor.
- Adheres to dress code, appearance is neat and clean.
- Completes annual educational requirements.
- Maintains regulatory requirements.
- Maintains and ensures patient confidentiality at all times.
- Reports to work on time and as scheduled, completes work within designated time.
- Wears identification while on duty.
- Attends annual review and in-services as required.
- Attends mandatory staff meetings and other discipline meetings and training as required.
- Complies with all organizational policies regarding ethical business practices.
- Actively participates in Performance Improvement (QAPI) and peer review.
- Represents the organization in a positive and professional manner in the community.
- Communicates the mission, ethics and goals of the organization.
- On call rotation required
Professional Requirements:
- Master’s Degree in ministry, counseling, social work preferred.
- Clinical Pastoral Education (CPE) training preferred.
- Certification required (i.e., APC, NACC, AAPC, NAJC, HCMA or NAVAC)
- Two (2) years or equivalent full time experience in bereavement counseling within the last three (3) years.
- Valid state driver’s license and reliable automobile, current automobile insurance and be willing to operate
- personal car necessitated by nature of job.
- Current health certificate/physical examination and TB testing.
