Job Description Position Overview:

Under direction, arranges and delivers appropriate chaplaincy care to patients, families, and staff, including scheduling and leading spiritual/religious services and providing spiritual and moral care, guidance, and support consistent with individuals’ beliefs and values.

Responsibilities:

Schedules and conducts religious services for members of the Chaplain’s faith. Administers rites and sacraments. Organizes and conducts religious educational programs. Initiates, plans and coordinates special projects and/or religious events. Breakdown and cleanup after events have taken place. Makes pastoral calls and visits. Counsels individuals and families on personal, moral and spiritual problems. Maintains liaison and cooperative relationships with social service agencies and other community organizations. Assist at Special and Centralized Events as assigned. Follows all facility-wide and departmental Policies and Procedures. Maintains and submits, on a timely basis, resident daily program participation attendance and statistics for group services as well as 1:1/bedside pastoral visits (Activity Pro Software). Follow facility policy regarding Outlook e-mail account. Check account daily. Responsible for the monitoring, maintaining/cleanliness of the assigned work area (i.e. Chapel) when conducting programs and services. Completes required documentation (Point Click Care, Epic) for religious services provided to residents and patients within the facility. Partakes in the Ethics Committee and acts as a resource to the committee, provides spiritual and moral perspective to ethical decision that focus on patient/resident family needs. Serves as an advocate for patients/residents needs and preferences, ensuring that their spiritual and ethical concerns are considered in the decision making. Works with the Clinical Team to address patient/resident or family member religious request and needs. Provides guidance to H3/ Wellness lead in spiritual, wellness/care, prayers and initiatives. Minimum Qualifications 1. A Bachelor’s degree from an accredited college or university in Divinity, Islamic Studies, Sacred Theology, or a related discipline, and a documented letter of endorsement from within the last six (6) months from a faith-based institution affiliated with the appropriate denomination (see note below), and
a. Documented completion of four (4) Clinical Pastoral Education (CPE) units (i.e., 1,600 hours of clinical pastoral training), or current and valid chaplain certification issued by the Association for Professional Chaplains (APC) Board of Chaplaincy Certification Inc. (BCCI), the National Association of Catholic Chaplains (NACC),

Neshama:

National Association of Jewish Chaplains (NAJC), or an equivalent; or
b. One (1) year of community-based experience as a recognized spiritual/religious leader.

Note:

A documented letter of endorsement may be substituted by an academic degree or chaplain certification endorsed within the last six (6) months by a faith-based institution affiliated with the appropriate denomination. The documented letter of endorsement or its substitute must be dated within six (6) months prior to the date of hire.
