Schedule:

EmployeeReferral Program Employee Discount Programs What You’ll Do:

Performs spiritual bereavement assessments of patients andfamilies/caregivers in the hospice program in a timely manner andensure that identified needs are addressed in the patient’s plan ofcare Provides direct spiritual care and bereavement counseling topatients and families/caregivers Demonstrates exceptional serviceto patients, families, referral sources, co-workers, and othercustomers. Assists team members as needed to ensure that this levelof service is consistently provided Provides accurate and timelydocumentation of patient services to reflect the plan of careAttends group meetings to facilitate coordination of care anddiscussion of interdisciplinary group involvement Providesconsultation, education, and support to the interdisciplinary groupon spiritual care and bereavement counseling Participates inContinuous Care and on call, if necessary Assists in supervision ofspiritual care volunteers when assigned to patient’sfamilies/caregivers as needed Assumes responsibility in directingthe development and implementation of the bereavement plan of careand the frequency of service delivery; make available bereavementservices for up to one year following the death of the patient

Qualifications:
