Full Job Description Description:

The Team Support Assistant is a role requiring strong self-motivation, organization, attention to detail, and the ability to manage multiple responsibilities simultaneously. This position is responsible for coordinating and managing daily activities for the Woodhaven Community Connections Day Program , both on-site and in the community. Duties include paperwork management, activity facilitation, budget oversight, establishing community partnerships and working in a direct support capacity as needed. The Team Support Assistant will ensure program participants have access to meaningful and enriching activities across areas such as recreation, volunteerism, culture, religion, health, education, skill development, and vocational exploration. This role requires collaboration with the Director and Assistant Director of Community Connections, particularly in areas related to program expansion, new admissions, and site development. The Team Support Assistant must maintain documentation and program operations in accordance with CARF standards.

Requirements:
Preferred :

Bachelor’s degree in human services, Vocational Rehabilitation, Recreation Therapy, Business, or related field

Required :

High school diploma or GED Two (2) years of experience working with individuals with developmental disabilities may substitute for a degree Excellent verbal and written communication skills Demonstrated maturity, flexibility, sound judgment, time management, and interpersonal skills Proficiency with computers and common office software Required Training and Certifications Completion of initial orientation, including: CPR, First Aid Crisis Prevention and Intervention (CPI) Universal Precautions Lifting techniques Medication Administration Training on relevant State and Federal regulations Annual renewal or as otherwise required Valid Missouri Chauffeur’s License within two (2) weeks of hire Acceptable driving record per Woodhaven Procedure No. 1-8-3 Knowledge of and adherence to CARF standards Essential Physical Requirements Non-sedentary work; must occasionally lift/move up to 40 lbs Frequent use of hands for typing, grasping, and repetitive motion Clear speech and hearing for communication Visual acuity for reading and evaluating data, computer work, and observing surroundings No regular exposure to adverse environmental conditions

Assistant Director of Community Connections Essential Duties and Responsibilities Note:

Responsibilities may vary depending on program needs. Plan and facilitate on-site and community-based activities for Day Program participants Build and maintain partnerships with community organizations and businesses to enhance volunteer and engagement opportunities Create and maintain a 12-month activity calendar and corresponding budget; submit updates monthly to the Assistant Director of Community Connections Lead efforts to expand community participation and enhance Woodhaven’s public presence Conduct monthly meetings with staff and participants to review program satisfaction and ensure alignment with individual preferences Meet all required paperwork and documentation deadlines , including monthly reviews, census data, fire drills, etc. Communicate proposed activities or initiatives to the Assistant Director of Community Connections for review and approval Ensure all participants experience a meaningful day through diverse and inclusive programming Respond to operational needs during programming hours, including direct service when necessary Monitor and manage the use of Community Connections funds; obtain approvals and maintain proper documentation for all purchases Serve as a liaison to Aktion Club and participate in monthly meetings Collaborate with the Finance Department to ensure appropriate spending and accurate financial records Maintain positive relationships with individuals supported, families, other agencies, and staff Represent Woodhaven in a professional and positive manner to stakeholders and community members Uphold the values of the organization and practice principle-centered leadership Perform other duties as assigned during emergencies or as directed by leadership Technology and Communication Maintain and appropriately use a company-issued cell phone for communication with staff, supervisor, and community partners Communicate effectively via email, phone, and in person to coordinate schedules, share updates, and support team collaboration
