Responsibilities :
Support the social service function in a long-term care and short-term rehabilitation nursing center. Work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related to psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans.
Qualifications :
High School Diploma or equivalent required. Post-secondary education or work experience in a social service or health related field a plus (such as social services, social work, patient registration, care coordination, or discharge planning). Must be familiar with Medical Terminology. Must be proficient in the use of personal computers (Windows, Internet, and Applications/Programs navigation)

Requirements:
ENTRY QUALIFICATIONS
Minimum high school diploma or equivalent required. One or more years experience in health care education/ marketing / insurance or commensurate educations preferred. Must be familiar with medical terminology. Proficient in the use of personal computer.
