The ideal candidate will assist the Social Services Director in planning and implementing programs that enhance the emotional, social, and psychosocial well-being of our residents.

Social Services Assistant Key Responsibilities:
Resident Support:

Assist residents and their families in adjusting to the facility environment and address emotional, social, and psychosocial needs.

Care Planning:

Participate in the development, review, and updating of individualized care plans to ensure the comprehensive well-being of each resident.

Documentation:

Maintain accurate and up-to-date records of social services assessments, progress notes, and care plans in compliance with federal and state regulations.

Resident Advocacy:

Serve as an advocate for residents by addressing grievances, resolving concerns, and ensuring residents’ rights are respected.

Discharge Planning:

Collaborate with the interdisciplinary team to coordinate discharge planning and ensure smooth transitions to home or other care settings.

Family Support:

Provide guidance and support to residents’ families, offering resources and education to help them understand and navigate the care process.

Program Development:

Assist in planning and implementing activities, support groups, and programs that promote social engagement and emotional well-being.

Compliance:

Ensure all social services practices adhere to state, federal, and facility regulations.

Interdisciplinary Collaboration:

Work closely with nursing, therapy, and administrative teams to provide holistic care to residents.

Social Services Assistant Qualifications:

High school diploma or equivalent required; Associate or Bachelor’s degree in social work, human services, psychology, or a related field preferred. Previous experience in social services, healthcare, or a similar role, ideally within a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting. Knowledge of federal and state regulations related to social services in SNF/LTC environments. Excellent interpersonal and communication skills. Strong organizational and time-management abilities. Proficiency in Microsoft Office and familiarity with healthcare documentation systems. Compassionate, empathetic approach to working with residents and families.
