We are seeking a Social Services Assistant to join our team and help meet the psychosocial needs of our residents, ensuring they are supported throughout their stay. Key Responsibilities:
Assist residents with psychosocial needs, such as adjusting to facility life, providing emotional support, and aiding with discharge planning. Conduct assessments and collaborate on care planning for new and existing residents as part of the interdisciplinary team. Maintain accurate and timely documentation in compliance with state and federal guidelines. Advocate for residents’ rights and help mediate any concerns or grievances from residents and their families. Coordinate social services interventions and referrals to external agencies (e.g., mental health, hospice, housing). Support discharge planning, including coordinating home services, equipment needs, and family or caregiver involvement. Actively participate in care conferences and collaborate with medical, nursing, therapy, and dietary staff. Plan and implement social programs and events to promote residents’ mental and emotional health. Uphold confidentiality and advocate for resident dignity and respect at all times.
Qualifications:
High school diploma or equivalent required; associate or bachelor’s degree in social work, psychology, human services, or a related field preferred. Prior experience in a healthcare setting, particularly long-term care or skilled nursing, is a plus. Knowledge of federal and state regulations related to resident care (e.g., OBRA, HIPAA).
