As a Social Services Assistant at Boulevard Rehab, you will work in conjunction with the Social Services / Admissions team to conduct intakes with new patients and residents, complete psychosocial assessments, develop and implement care plans, and assist with discharge coordination and post-discharge referred. Qualifications :

BA in Social Work or Sociology Experience in MDS. Successful completion of a Bachelor of Arts Program required. Experience in Social Services Assessments. Must be PCC proficient with prior experience using Windows, Internet Based programs and other software and applications as required.
