Key Responsibilities:
Assist in planning and providing social services for skilled nursing residents Support residents and families with counseling and care transition planning Collaborate with interdisciplinary care teams (nurses, therapy, etc.) Maintain accurate documentation in compliance with regulations Help identify psychosocial needs and deliver appropriate interventions
Qualifications:
Previous experience in a social services or case management role required Long-term care or skilled nursing facility experience strongly preferred Must meet all state-specific certification/licensure requirements Strong communication and documentation skills .
