Interviews residents and families periodically to obtain social history and to determine social needs Develops the preliminary and comprehensive assessments of the social need of each resident Develops the residents social plan of care and discharge plan and ensures that social service personnel follow the plan of care Ensures that the social needs of all residents are provided for with the highest quality of care
