Social Services Assistant:
Planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families Evaluating and being keenly aware of residents’ medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan Coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families/responsible parties and the treatment team Maintaining residents’ rights at all times while liaising between all parties involved Coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents Ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights
