We look forward to meeting you! Responsibilities of the Social Services Assistant include, but are not limited to, the following: Facilitate care plan meetings & discharge planning. Implement History and Assessment, Advanced Directives, and End of Life Planning. Audit psychology paperwork, hospital discharges, room changes, PASRR, and Advanced Directives. Completing all required documentation in the medical record ( EMR) Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication, and Organizational Skills Demonstrates judgment of safety and confidentiality issues Strong verbal and written communication skills Strong customer service focus required Knowledge of the community resources Good working knowledge of long term care regulations including documentation requirements First Shift 7am-3pm Benefits for the
