Job Description:

Complete comprehensive psychosocial evaluations to understand residents’ emotional, social, and spiritual needs, as well as their family dynamics and support systems. Collaborate with patients/residents and their families to provide support and resource information. Respond to issues identified by patients/residents and their families. Facilitate patient/resident transfer within the nursing center to ensure a seamless transition and adjustment. Assist in transporting resident/patients to and from outside physician appointments as needed. Document patient/resident transfers and adjustments accordingly. Participate in the health care decision-making process under the direction of Social Services Director.

Job Requirements:

High School Degree Required Associate degree in human services. Minimum of two years’ experience in long-term care.
