Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 1. Interviews Residents to assist in the development of a comprehensive social history and psychosocial assessment that includes the Resident’s problems, strengths and preferences, including cultural, religious and ethnic background and implications for the care plan. 2. Orients Residents and families to the facility, its service limitations and Residents’ Rights. 3. Assist with the coordination, intake, admission and Resident transfer within the facility. 4. Coordinates the inspection of Resident’s belongings to ensure they are properly labeled and inventoried. 5. Assists Residents with concerns, completing concern form and addressing the concern in a timely manner. 6. Provides 1 to 1 individual visits with Residents when requested, on a regular basis. 7. Assists Residents to Activity Programs of choice. 8. Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons. Follows established safety precautions when performing tasks and using supplies or equipment. 9. Answers and responds to call lights promptly and courteously when working in Resident care areas. 10. Maintains privacy, comfort and dignity of Residents and interacts with them in a mannerthat displays warmth, respect and promotes a caring environment. 11. Communicates and interacts effectively and tactfully with Residents, Visitors, families, peers and supervisors. 12. Assists with answering phones and transferring calls/paging when needed. Required Education and Experience Experience interacting with the elderly and Residents with Dementia is required. Counseling/Social Work or education is a plus but not required. #YAD123
