Carolina Therapeutic Services First (CTSF) is seeking a Qualified Professional with experience in Community Support Team (CST), TCL housing, and other housing programs to join our dedicated team. Your Role:
As a Qualified Professional, you will play a vital role in providing support and guidance to individuals with complex needs. You’ll work as part of a Community Support Team (CST) that operates 24/7/365, ensuring continuous care and assistance for individuals facing mental health and substance use challenges.
Key Responsibilities:
Complete and maintain essential documentation, including: DLA-20 Service Authorization Requests Person-Centered Plans (PCP) Coordinate, access, and deliver Community Support Services in alignment with wraparound philosophy. Assist individuals in community integration and adjust service intensity as needed. Maintain a 12:1 beneficiary-to-staff ratio with a maximum team caseload of 48 individuals. Provide psychoeducation, crisis intervention, and relapse prevention strategies. Conduct functional needs assessments and monitor client progress. Assist beneficiaries with housing searches, financial aid, and household setup. Engage landlords, draft reasonable accommodation letters, and address housing crises. Help individuals develop daily living skills to stabilize housing and promote independence. What You Bring to the
Team:
Knowledge & Expertise:
Strong understanding of Community Support Team policies, clinical diagnosis, treatment approaches, and
MH/DD/SAS
(Mental Health/Developmental Disabilities/Substance Abuse Services) principles. Familiarity with the Diagnostic and Statistical Manual (DSM) and state service definitions. Proficiency in record-keeping, filing systems, and communication skills.
Education, Training & Experience:
Bachelor’s degree with 4 years of experience in a human services-related field under a
QP. OR 30
semester hours in a human services field (e.g., psychology, social work, counseling, nursing, education, occupational therapy, recreational therapy) with at least 1 year of experience serving the target population.
Work Schedule & Requirements:
